Enrollment & Key Terms
Here's everything to know before you sign up for in-house savings at Dental Beauty.
- A one-time existing patient enrollment fee of $45 is due at the time of enrollment.
- New patients will pay a $95 one-time enrollment fee.
- Plan terms follow a 12-month agreement – plans auto-renew annually unless canceled at least 30 days before renewal.
- Patients can choose from a $35–$65/month auto-charge or pay annually in full. The total cost is the same either way.
- Plans are non-transferable – savings apply to the named enrolled member(s) only.
- This plan is not insurance and cannot be submitted to any insurance carrier.
- Cancellation policy: Cancel within 30 days for a prorated refund.
- Enrollment fees are non-refundable once activated.
Terms & Conditions
All plans are 12-month agreements that renew automatically unless canceled at least 30 days before the renewal date. This plan is not insurance and cannot be billed to any carrier. Memberships canceled within 30 days of enrollment are eligible for a prorated refund on unused services.
The Dental Beauty in-house membership is available to patients without active dental insurance. Unused benefits do not roll over. The Children's Plan is for patients under age 14. The periodontal plan requires a formal periodontal diagnosis. Non-transferable.
